If you would like to order patient demonstration samples from our EPIMAX® range, for your patients, please complete the forms below.
Please note, if you would like to receive samples of the the full EPIMAX® range (including EPIMAX® Eyelid Ointment and EPIMAX®Gentle Wash) you will need to complete both of the forms below.
Please advise your patients to always read the label.
